Should smokers be refused surgery? Options

There is a growing argument that Doctors should refuse treatment to people who smoke, or at the very least offer their services to non-smokers rather than smokers. What do you think? Is it ethically wrong to refuse treatment to someone, even if they smoke? Or should smokers simply grin and bear it?

I think anyone who needs a serious operation such as a heart transplant or any organ transplant has a moral right to give up smoking and lead a healthier lifestyle. They are lucky to get help. Those who abuse the medical treatment they receive - while others miss out - should feel ashamed of themselves.
